Its central theme is that the widely differing perspectives of those who plan, fund, deliver, receive, record and administer care may each be valid in particular contexts, and a holistic approach to quality must incorporate these multiple perspectives. This paper recognises the plurality of perspectives that are already beginning to exist. It begins by describing the historical and political context for the current drive for quality in UK primary care. It then illustrates the notion that the core features of general practice remain difficult to define and measure and argues that the reflective practitioner should be conscious of the tendency of quality assessment to measure the easily measurable and exclude the essence of good general practice. A number of individual perspectives on quality are then explored. The different potential approaches for using quality initiatives as levers for change in general practice are then discussed, with the authors describing their own work to develop a methodology for defining, measuring and improving quality at a local level and arguing that extensive dialogue with key stakeholders is necessary when standards are being set. Yet the subtle and intangible elements that underpin a strong therapeutic relationship are difficult to define and to measure. To inform its work, the Inquiry into the Quality of General Practice in England commissioned this discussion paper on the importance of the therapeutic relationship to the overall quality of general practice care.
