This report argues that an independent board would be limited in its ability to achieve these aims. It suggests a number of alternative options to an independent board that build on the increasing operational independence the recent reforms have offered. These options would help secure a better balance between public accountability and local autonomy within a more devolved health care system. Does the health service need to be freed from political interference, and allowed to get on with the job of delivering care as clinicians and managers think best, within a broad, agreed policy framework?  \"Government and the NHS - time for a new relationship?\" probes the conceptual and practical challenges - as well as the potential benefits - of arm's-length governance for the NHS.  It reviews past arguments, and considers how a contemporary 'arm's-length' NHS agency, accountable to Parliament, could work with government to improve health care.  The paper concludes that this new relationship could help make the NHS more accountable, encourage a more transparent and inclusive approach to setting national health policy, and create greater ownership of targets by NHS staff. The traditional accountability of NHS hospitals to the Department of Health has been replaced by a new accountability of foundation trusts to their members and their elected governors, and to an independent regulator called Monitor. Foundation trusts enjoy new freedoms to control their own affairs, so it is important that the new governance arrangements work well; if not, NHS hospitals will cease to be accountable. This paper sets out different roles that foundation trust governors might usefully play; these roles are intended to provide a framework for debate between boards of governors and boards of directors. It is vital that a clear agreement over respective roles is reached within each foundation trust.
