Its aim is to find ways in which the debate on funding long-term care can move forward. This report gives the results of an interview survey of 100 people undertaken in June and July 2000. They were asked questions about their financial circumstances, and about their ability to pay more towards their health and social care costs, now or in the future. Results suggest that present and future pensioners on average and below- average incomes are struggling or will struggle to either meet increased costs of care services, or invest in the long-term savings or insurance products which the government is considering introducing. The results also confirm that people do not understand how long-term care is financed. The results also suggest that although adults on average and below average household incomes might be expected by the government to invest in supplementary pensions and save for their own long-term care needs, they cannot realistically be expected to do so. For the first time, Government ring-fenced funding to enable carers to take a break from caring as part of its overall strategy to support carers, 'Caring about Carers'. Local authorities were expected to show at least 75 per cent of the grant in the first year, totalling £140 million over three years, was to be spent on additional services providing a break. The picture emerges of some interesting examples of work to improve the range and flexibility of services offering a break but overall it is very patchy. There are worrying signs of a policy implementation gap in the first year where a large proportion of the grant is not finding services which directly offer carers a break. Fifty-six systematic reviews covering a variety of disabilities, health problems, diseases and client groups were identified through a literature search. Data was extracted from them in a systematic way and is presented as an evidence table. Two main types of intervention have been evaluated; packages of care and simple interventions, but further important detail about components of the different interventions is often lacking. The report suggests that enough is now known from a clinical effectiveness perspective to move forward significantly in the organisation and provision of rehabilitation. Policies have been developed in health care, education and social housing that aim to give users a greater choice of publicly or privately owned providers, and to ensure that a proportion of the funding to providers follows those choices. The government believes that this will put pressure on providers to improve their services and make them more responsive to users' needs. Although the scope of 'choice' in health care is potentially large (including choice of treatment) this briefing looks at the development and impact of reforms to give patients a choice of provider for their planned hospital care. It is this aspect of choice that has received the most attention so far and has been a key part of the government's wider reform programme for improving quality and efficiency in the NHS in England.
