But which variations are acceptable or warranted – for example, variations driven by clinical need and informed patient choice – and which are not? The important question is how to promote ‘good’ variation and minimise ‘bad’ variation. Variations in health care: The good, the bad and the inexplicable explores the possible causes of variation, shows the different ways in which variations can be measured, and analyses variations by PCT in rates of elective hospital admissions for selected procedures. The causes of variation are complex and inter-related – they may be affected by, for example, differences in geographical patterns of illness, differences in clinicians’ behaviour, the effects of incentives in the financing of health care. These causes are mapped and discussed. The data itself is not always easy to collect and analyse. The report outlines the different ways in which variations can be measured and how these measures can be adjusted for need – age and gender, economic and social characteristics. To illustrate geographical variation, the report analyses rates of elective hospital admissions for common procedures across PCTs – eg, hip replacement, cataract removal, tonsillectomy. It also breaks the information down into : variations in common, clinically effective procedures, trends in variation, variation in day case rates, variation in low effectiveness procedures, variation in pre-operative health, preference-sensitive variations.  Even after allowing for legitimate factors that affect rates of surgery, significant variations were found between PCTs – even among common interventions known to be effective. Policy-makers, commissioners and providers will benefit from the recommendations made for tackling this problem.  The first step is to ensure that data is collected and analysed systematically. The Department of Health’s Atlas of Variations may help to focus future reporting of variations; it would be good to extend this to cover the new population groups of GP commissioning consortia. Thereafter the recommendation is that a programme is established to identify causes of variation at local level and to assess which variations and causes have the most impact on equity, effectiveness, efficiency and patient health outcomes. The impact of the current reforms is considered: will GP consortia be able to implement shared decision-making as a way of establishing more appropriate level of warranted variation? Surveys consistently report high levels of trust in GPs and good levels of patient satisfaction with the services they receive in general practice. However, other than data available through the Quality Outcomes Framework and the GP Patient Survey, very little information is published on the quality of care in general practice. It was for this reason that The King’s Fund set up, in April 2009, an independent inquiry into the quality of general practice in England. The aim of the inquiry, which was conducted by an independent panel of experts and chaired by Sir Ian Kennedy, was to help to support the work of general practice and to provide a guide to ensure that quality is at the heart of the service that it offers to patients. Improving the quality of care in general practice is the report of the inquiry; it represents the most extensive review of quality across general practice carried out in recent years. Its work was informed by specially commissioned research and analysis of routinely available data across a range of aspects of general practice including: core elements of day-to-day practice - for example, diagnosis, referral and prescribing; non-clinical aspects of quality – for example, access to care and patient engagement; and areas where the role is shared with others – for example, maternity and end-of-life care. The recent root -and-branch reforms of the NHS also have profound implications for the future of general practice, in particular the dual role it is being asked to play as both commissioner and provider of care that will mean GPs taking on responsibility for costs as well as outcomes of care. The timing of the inquiry’s report is therefore propitious. General practice is at centre stage and quality must be the watchword. In terms of the current quality of care, the inquiry concluded that the majority of care provided by general practice is good. However, there are wide variations in performance and gaps in the quality of care that suggest there is significant opportunity for improvement. Practices need a lot of support to encourage them to seek out and address variable performance, including: appropriate data and information; skills development; protected time; and appropriate rewards for excellence (as well as consequences for poor performance). The government has recently undertaken an evaluation of the 2009/10 quality accounts and set out their expectations for 2010/11. The King’s Fund undertook an independent analysis of a sample of quality accounts produced by a range of providers – acute trusts, mental health trusts, independent providers. The analysis focused on three particular aspects of the quality accounts: the choice of quality measures used to review performance, their reliability and presentation; the quality of the data and how far providers have participated in national clinical audits and confidential enquiries; how providers have reported patient and public feedback and  involvement of local stakeholders and what external comments have focused on.  This paper includes examples from 2009/10 quality accounts that show how the content and the presentation of information can vary. Based on these examples, we make recommendations for providers when they are developing and writing a quality account. We also have recommendations for policy-makers for future guidance on quality accounts; on data quality; on clinical audits and confidential enquiries; and on external scrutiny. We conclude that, fundamentally, quality accounts are so varied because they are having to provide commentary on a wide range of services, are serving a broad range of audiences and are also attempting to meet two related, but different, goals of local quality improvement and public accountability. The future for public accountability needs to focus more on the centralised provision of standard, consistent and comparable measures, published in forms that enable interpretation and comparison.
