As the population in England ages, growing numbers of patients will need help with managing complex, multiple conditions over sustained periods. Apart from the burden of ill-health, treating these conditions is likely to cost the NHS far more than elective surgical procedures. This is a summary of a report which offers an analysis of five managed care organisations across the United States and asks what lessons or transferable models might emerge for health care in England. It asks how the wider economic and social context affect how they work, examines the clinical processes they use to manage chronic disease, and puts forward recommendations for the future NHS. Most of these people are leading full and active lives with only occasional contact with health professionals, by altering drug doses and adapting their lifestyles in response to subtle changes in symptoms. This paper reviews patients' perceptions about managing their own conditions, and identifies how primary care trusts and other health and social care providers can support them, in line with their individual needs.  Three key areas for service development are: improving health professionals' skills to help patients manage their own conditions; improving the provision of information about long-term conditions and the local services available; and increasing the flexibility of services to fit in with patients' other commitments. Five outcome measures were considered: hospital admissions, use of emergency departments, length of stay, functional health status and costs. The review concludes that while there is come support in the literature for case management as a method of reducing the use of hospital beds,  the evidence is not conclusive. The variety of case management arrangements under investigation makes it hard to generalise results to local NHS settings. The review's recommendations are that: PCTs need to be clear about the needs of the population at whome case management is targeted; local discussion is needed about whether case ,management is best developed by adapting existing services or whether new arrangements should be put in place; case management should be developed in close collaboration with social care providers; PCTs need to ensure that services are in place for people with less severe illness and all case management initiatives should be evaluated.
