Stronger proposals are required for improving the quality of inpatient care and services for people with complex needs. The barriers to implementing the approach described in New Horizons are considerable, and include challenges in terms of the skills and attitudes of clinicians; commissioning competencies; and multi-agency working. Further detail on the strategies and levers that will be used to overcome these would be beneficial. The need for quality improvement in primary mental health care goes well beyond what will be achieved by the Improved Access to Psychological Therapies programme. Attention should be paid to the models of care used in general practice, as well as to the mental health skills, knowledge and attitudes of primary care professionals.
