While not an official public-private partnership (PPP) there has been, in effect, an implicit PPP for pharmaceutical research and provision. This partnership has been an undoubted economic success. However, the interests of users of the health care system do not always coincide with what will be most profitable for the pharmaceutical industry. This publication argues that, because it is focussed on new medicinal drugs, the implicit PPP for pharmaceuticals neglects other research areas that may be beneficial for promoting people's health. It also omits key user groups and is therefore in danger of failing to meet the health needs of the very people it is supposed to be benefiting. It highlights where changes can be made to ensure there are sufficient incentives for research to tackle neglected areas that could be of value for improving health. It points the way forward towards a more transparent process with an emphasis on user involvement, and sets out the agenda on which future arrangements between the industry and the government should be negotiated. The Inquiry was established to find out whether the care system operating in 2004 was meeting the needs and preferences of older Londoners who require care and whether there will be sufficient care services of the right design and quality to meet the needs of older people in London in 20 years. The key finding from the Inquiry is that there are major shortcomings in the current system which disadvantage older people and their carers. They experience: restricted access to care and practical support; limited choice and control over care services; being put at risk from untrained and unqualified staff;  and hardship caused by inadequate funding and controversy about who pays for long term care. The Inquiry calls for: investment in market development; reform of social policies; and mobilisation of more public and private resources for the care of older people. Yet the subtle and intangible elements that underpin a strong therapeutic relationship are difficult to define and to measure. To inform its work, the Inquiry into the Quality of General Practice in England commissioned this discussion paper on the importance of the therapeutic relationship to the overall quality of general practice care.
