It sets the scene for the 'Public Health and Public Values' project at the King's Fund. The project aims to engage Londoners in a public debate about how public values relate to current public health issues in the capital. Funded by the NHS Executive's London Region, the project will inform the development of the 'Health Strategy for London'. It aims to provide a deeper understanding of what Londoners feel are important principles for public health and policy decision-making. In Chapter 1, the meaning of public health and the principal strategies available for achieving its goals are reviewed. In Chapter 2, the wide range of ethical and value-based issues found in the public health literature are summarised. Seven specific values which emerge from this analysis are set out in Chapter 3. In Chapter 4, the conflicts and synergies between these values are reviewed. In this report the authors warn that the NHS has ignored many of the problems facing nurses because it has been able to rely on their commitment to its ideals and to its work. That loyalty, the report argues, should not be assumed to be indefinite. Nurses are highly dissatisfied with many areas of their work: low pay, bleak conditions, constraints on resources and lack of control over what happens around them. Many, particularly nurses from black and minority ethnic groups, suffer discrimination at work. Inflexible working times, problems in obtaining affordable housing, the grading system and the long stream of organisational changes to the NHS are also causes of complaint. This report argues that the NHS needs to make improvements on all these fronts if it is to recruit and retain nurses successfully. It is now widely acknowledged that improving health involves a range of different activities by many players on many fronts.  Government policy, set out in the White Paper, 'Saving Lives: Our Healthier Nation', identifies specific targets for reducing avoidable deaths from cancer, coronary heart disease and stroke, accidents and suicides. It sets broader objectives for improving health and reducing health inequalities. It also stresses the importance of individuals, local communities and national government working together to achieve these goals. The government has emphasised its commitment to 'evidence-based policy-making' and wants to link public investment to knowledge of 'what works'. However, it is notoriously difficult to assess the effectiveness of many forms of public health activity, especially those relating to the underlying causes of health and ill health. Conventional approaches to evidence are insufficient and unhelpful. There is not enough evidence of the types required, and what does exist is too narrowly defined and biased towards a medical model of public health. A common approach, which all parties can endorse, is needed. a new shared framework is proposed, within which a more inclusive and appropriate evidence base can be built up over time. A wide range of services are implicated. There is clear evidence that some services have operated explicit age restrictions which have little justifiable clinical basis. Age discrimination is more often covert and subtle and is implicit in a general lack of priority for older people's services. Discrimination is sometimes difficult to separate from other issues around gender, poverty, ethnicity and the way in which people with disabilities and long term illness are treated. This paper was produced as part of a wider project on age discrimination being undertaken at the King's Fund. Future work will explore the options for tackling ageism in health and social care.
