Views of the young people in question are considered, as well as the policy and practical responses to the health needs of this group. The interim evaluation suggests however that these cases do not indicate improved effectiveness when compared to a control group using existing services, and the service has been of limited use to outside agencies. Under One Roof has tested two other models for developing 'joined-up' services for single homeless people. It has succeeded in piloting a new type of multi-agency panel focusing on the needs of vulnerable rough sleepers. It has also identified a new approach to 'generic assessment' which requires further development. There have been identifiable improvements in joint working at both strategic and operational levels. As in the UK, governments are finding that it is a complex and difficult arena in which to introduce change. Direct and indirect regulation are both options which can be used. Under pressure from their citizens, governments face a dilemma. Public trust in the authority of doctors needs to be maintained or restored if the profession is to continue to deal with the demand-supply mismatch in the quantity and quality of health care. Yet to rely on the profession to manage its own process of organisational change to achieve that trust is gambling against history. On the other hand, the introduction of regulatory reform for which the state takes responsibility means that it, rather than the profession, then becomes the target for citizen discontent with the standards of health care. An evaluation of the  first three sites is still underway and, although preliminary results show high satisfaction among service users, it is too early to draw conclusions about the impact of NHS Direct on patient health and on other NHS services. Despite this, the service is being rapidly expanded and two London sites were among the second wave of NHS Direct sites launched in spring 1999.By the end of 2000 the service will cover the whole country.  This report describes an interview-based study of the implementation of NHS Direct in west London and south London. It aims to draw lessons from the London experience in order to inform the development of other NHS Direct services and to identify possible future roles for this nation-wide telephone
