The analysis of hospital episode statistics was undertaken in order to establish whether the action taken by these TPPS to change emergency hospital activity had any impact. By the overall measure of impact used in the study, 69 per cent of the pilots were successful. The report distinguishes between multi-practice and single-practice TPPs, because the multi-practice pilots offer greater insight into the issues faced by primary care groups (PCGs). The commissioning of emergency hospital services was the pilots' raison d'etre. The finding that only a minority of the pilots pursued objectives of reducing acute hospital emergency activity, indicates that the challenges to be overcome are considerable. The total purchasing experience has implications for PCGs and primary care trusts attempting to manage their use of, and expenditure on, acute hospital emergency services. Although they were volunteers and comprised fewer practices than PCGs, these pilots demonstrate that general practice can influence the use of emergency hospital services. The specific aims were to find out the range of clinical practice, and discover any difficulties that dentists and patients encounter, including categories of vulnerable patients for whom there are particular ethical and legal issues in gaining consent. At the outset, nine stages in obtaining informed consent were proposed: the initial introduction, explaining the dental problem, outlining treatment options, communicating risks and benefits, estimating time and cost, inviting questions reaching mutual understanding, confirming choices, and indicating consent. The working hypothesis is that these stages are all addressed in the dental consultation. The authors conclude that the study shows that the principle of informed consent is generally acknowledged by dentists to be of value; however there is wide variation in current practice, with individual stages of the consulting process being identified but not always used and not forming a coherent pattern. This outcome had been anticipated in an earlier King's Fund paper (Hamblin et al., 1998) which argued that not only was the government pursuing the wrong objective, but also that the policies it adopted to achieve that objective were misconceived. This report builds on the earlier framework but extends it in four main ways: it considers all the stages through which NHS patients must pass to gain access to elective care; it expands the empirical description of how the elective care system works; it considers the role of waiting in the elective care system, specifically how it functions as a rationing device; and it sets out in more detail two broad policy options for 'solving' the waiting list problem. It describes how policy towards elective care has developed over the years and sets the scene describing the numbers waiting, the times they wait and recent changes in both. The report then considers a number of issues raised: why the numbers of patients waiting rise even when activity increases, why waits vary between different patients, why waits matter to the system and to the individual, and why waits vary from one area to another. The report goes on to consider how the broad objectives of any area of public policy - efficiency, equity and accountability - apply to elective care, and concludes by examining a range of policy
