The analysis presented here considers eleven of the case studies and links findings with other sources of information on the TPPs, in particular, the collection of monitoring data undertaken at all the first and second wave TPPs by postal survey and telephone interviews in 1997/8. The case study analysis can be arranged into five groups: TPPs as organisations; tools and levers for achieving change; the scope and nature of benefits/outcomes for patients and populations; the position of TPPs within local commissioning systems; and management capability - costs and issues of sustainability. From this analysis the report draws the following conclusions about the implications for future primary care-based purchasing organisations. In operational terms, the characteristics associated with achievement were: the ability to hold and use budgets; the availability of adequate management resources; good relationships with external agencies, particularly the health authority; effective management systems; the presence of product champions both within the TPP but also in HAs and trusts; a strong dynamic executive management team; the inclusivity of stakeholders; the engendering of a collective responsibility and corporacy within the TPP; and a positive national and local context. In terms of service development, where there is vested local interest or enthusiasm, there tends to be more success than when the objectives are more strategic. The results also suggest that although adults on average and below average household incomes might be expected by the government to invest in supplementary pensions and save for their own long-term care needs, they cannot realistically be expected to do so. and changes in demographics and the rise of female employment suggest that current levels of informal care will be difficult to maintain. These combined factors point to the need for an urgent review of policy to address the issues of dependence in the 21st century. For policy makers and service providers, the King's Fund recommends the establishment of the Forum for Autonomy, Choice and Innovation to establish new partnerships and approaches and to develop better methods of gathering and analysing the evidence they need to use their resources to best effect. The Forum's three initial programmes will gather evidence on: consumer dependence (its nature, scale and patterns), responses to dependence (the availability and effectiveness of equipment, products, services and policies for restoring autonomy) and human and economic effects of dependence (the penalties for the stakeholders when consumers remain dependent instead of regaining their autonomy). For the first time, Government ring-fenced funding to enable carers to take a break from caring as part of its overall strategy to support carers, 'Caring about Carers'. Local authorities were expected to show at least 75 per cent of the grant in the first year, totalling £140 million over three years, was to be spent on additional services providing a break. The picture emerges of some interesting examples of work to improve the range and flexibility of services offering a break but overall it is very patchy. There are worrying signs of a policy implementation gap in the first year where a large proportion of the grant is not finding services which directly offer carers a break.
