Failure to address avoidable ill health will put huge pressures on the future NHS while deepening health inequalities. Doctors remain 'professionals' but the traditional image of what this means in practice is increasingly outdated. While individual doctors remain highly trusted, the profession as a whole needs to demonstrate better its duty to serve patients' interests and to show that it can respond to changing public and political expectations. To sustain and build trust in the profession, doctors need to ensure that their practice reflects the behaviour expected of them. This paper examines recent challenges to the profession, explores how a new compact between key stakeholders might be formed, and offers a way forward to redefining medical professionalism for the future. It will be wound up in April 2004, when its responsibilities will be passed to the Commission for Healthcare Audit and Inspection (CHAI). This three year study analyses CHI's methodology, activities and impact. The aim was to use CHI's experience to illuminate the challenges and dilemmas of inspection in the NHS and the focus was on reviews of acute trusts. It concludes that CHI is an example of successful institution-building, and that its replacement by CHAI should be seen as policy adaptation. Its problems with methodology largely reflect the fact that it has to work within the framework of clinical governance. CHAI's remit to look at the quality of care rather than clinical governance as such represents a rebalancing of the regulatory task, which will give the new body greater freedom to develop its strategy and methods. These priorities were: free long-term care for older people; better pay for NHS staff; shorter waiting times for cancer and heart care; improved accident and emergency departments and cleaner hospitals. Overall, the research paints a largely positive picture with improvements in most areas. However, it is still too early to judge whether the extra resources the government has put into the NHS are delivering value for money and whether better health outcomes for more people will result. The research also illustrates sharp contrasts between approaches and outcomes across the four countries of the United Kingdom.
