The General Medical Services contract introduced a pay-for-performance scheme known as the Quality and Outcomes Framework (QOF). Although QOF was not developed explicitly to address public health issues or reduce health inequalities, there has always been interest in whether it could contribute to improvement. The National Institute for Health Research Service Delivery and Organisation programme commissioned The King’s Fund, together with the London School of Hygiene \u0026 Tropical Medicine, to explore the impact of the QOF on public health and health inequalities.  Our research aimed to examine whether the GMS contract and, in particular, the QOF was contributing to improvements in public health and reductions in health inequalities; the focus was on health inequalities as determined by income deprivation. The researchers on the QOF used routine data to assess the extent to which QOF encouraged activities that contributed to improvements in the health of the population. They also conducted interviews with staff from PCTs and practices to understand how they had responded to QOF and what impact they felt it had had on their ability to improve health and reduce inequalities. Impact of Quality and Outcomes Framework on Health Inequalities summarises the findings of that research, specifically on: incentives for prevention and public health activities, differences in performance by deprivation, impact on population health, practice characteristics, exception reporting (allowing certain patients to be excluded from the performance figures) and case finding (actively seeking to identify new people at risk). The current government has identified cancer survival rates as an area for improvement and the cancer strategy commits to saving an additional 5,000 lives by 2015, but how can these improvements be achieved? How to improve cancer outcomes, published in partnership with Cancer Research UK, considers the existing differences in cancer survival rates between countries and discusses the reasons for these variations including: stage at diagnosis and diagnostic delay; treatment factors; patient factors; and tumour biology and physiological/biological factors. The authors suggest that the most plausible drivers for improved survival rates are: diagnosis at an early stage, including through effective screening programmes, access to optimal treatment, improvements in the management of older people with cancer. To achieve these improvements, the NHS and public health need to work together to diagnose more cancers at an early stage and GPs need to use information about their referral rates and use of diagnostics to understand how their performance compares with others. The government has recently undertaken an evaluation of the 2009/10 quality accounts and set out their expectations for 2010/11. The King’s Fund undertook an independent analysis of a sample of quality accounts produced by a range of providers – acute trusts, mental health trusts, independent providers. The analysis focused on three particular aspects of the quality accounts: the choice of quality measures used to review performance, their reliability and presentation; the quality of the data and how far providers have participated in national clinical audits and confidential enquiries; how providers have reported patient and public feedback and  involvement of local stakeholders and what external comments have focused on.  This paper includes examples from 2009/10 quality accounts that show how the content and the presentation of information can vary. Based on these examples, we make recommendations for providers when they are developing and writing a quality account. We also have recommendations for policy-makers for future guidance on quality accounts; on data quality; on clinical audits and confidential enquiries; and on external scrutiny. We conclude that, fundamentally, quality accounts are so varied because they are having to provide commentary on a wide range of services, are serving a broad range of audiences and are also attempting to meet two related, but different, goals of local quality improvement and public accountability. The future for public accountability needs to focus more on the centralised provision of standard, consistent and comparable measures, published in forms that enable interpretation and comparison. But what sort of leaders does the service need?  Does the model, prevalent in public service over recent years, of the ‘hero’ chief executive still hold sway? The King’s Fund set up a commission on leadership and management in the NHS with a brief to: take a view on the current state of management and leadership in the NHS, establish the nature of management and leadership that will be required to meet the quality and financial challenges now facing the health care system, recommend what needs to be done to strengthen and develop management and leadership in the NHS. The commission invited submissions from individuals and organisations with an interest in management and leadership and commissioned papers from experts. 'The future of  leadership and management in the NHS: no more heroes' reflects the conclusions of the commission’s work. The conclusions challenge some of the negative attitudes towards managers, challenges current plans for major reductions in management and administration costs. The commission believes that the NHS needs to move beyond the outdated model of heroic leadership to recognise the value of leadership that is shared, distributed and adaptive. In the new model, leaders must focus on systems of care and not just institutions and on engaging staff in delivering results. There is a clear message that the NHS will be able to rise to the financial and quality challenges it is faced with only if the contribution of managers is recognised and valued. It is also essential that the number of managers in the NHS, and expenditure on management, is based on a thorough assessment of the needs of the health service in the future rather than arbitrary targets and is supported by continuing investment in leadership development at all levels.
