The Routes project, a simulation exercise created by Loop2, allowed senior managers and thought leaders from across the NHS, local government and the third sector to test out a remodelling of the health and social care system in a safe learning environment. Routes for social and health care: A simulation exercise focuses on Crafton – a hypothetical metropolitan borough in central England – and sets up a number of routes to managing change. During the simulation two contrasting viewpoints emerged. One group took a strongly managerial perspective that recognised the need for immediate decisions to reduce expenditure, while putting in place the conditions for a radical transformation in the way that people receive care and support in the longer term. The other group took a more ‘devolutionist’ approach, arguing that the financial challenges across health, social care and housing could not be managed through a top-down approach; they proposed a radical acceleration of personalisation and community engagement. Clinical leadership will be critical as the service faces this challenge. The King’s Fund has a wealth of experience in developing the skills of leaders in the NHS, and is constantly adjusting its approach to meet the needs of those leaders. With this in mind, we conducted a survey of clinical and medical directors to find out what skills they believed they need to face these challenging times. “Leadership needs of medical directors and clinical directors” includes the following findings. Both medical and clinical directors embrace the quality agenda articulated by Lord Darzi in High Quality Care for All (Darzi 2008), and are committed to preserving it. Both medical and clinical directors are eager to work with their managerial colleagues as equal partners to maintain clinical quality, ensure patient safety and improve patients’ experience in the face of reduced resources. They see an urgent need for clinicians and managers to share a single mindset on maintaining quality efficiently. Clinical directors believe that they are largely cut off from the decision-making and planning processes, and view this as a critical challenge that must be overcome. But what sort of leaders does the service need?  Does the model, prevalent in public service over recent years, of the ‘hero’ chief executive still hold sway? The King’s Fund set up a commission on leadership and management in the NHS with a brief to: take a view on the current state of management and leadership in the NHS, establish the nature of management and leadership that will be required to meet the quality and financial challenges now facing the health care system, recommend what needs to be done to strengthen and develop management and leadership in the NHS. The commission invited submissions from individuals and organisations with an interest in management and leadership and commissioned papers from experts. 'The future of  leadership and management in the NHS: no more heroes' reflects the conclusions of the commission’s work. The conclusions challenge some of the negative attitudes towards managers, challenges current plans for major reductions in management and administration costs. The commission believes that the NHS needs to move beyond the outdated model of heroic leadership to recognise the value of leadership that is shared, distributed and adaptive. In the new model, leaders must focus on systems of care and not just institutions and on engaging staff in delivering results. There is a clear message that the NHS will be able to rise to the financial and quality challenges it is faced with only if the contribution of managers is recognised and valued. It is also essential that the number of managers in the NHS, and expenditure on management, is based on a thorough assessment of the needs of the health service in the future rather than arbitrary targets and is supported by continuing investment in leadership development at all levels. An earlier series of events, ‘Do Doctors Have a Future?’ ran from May 2006 to April 2007\nand looked at the way that doctors viewed the challenges that they face.","Acquiring and ensuring a professional approach is a key part of becoming a doctor. But how can doctors define and adopt this professional attitude and ensure that it is kept up to date in the same way as their clinical skills? And how should professionalism adapt and change to reflect changing social values and attitudes?  These questions were explored by The King’s Fund and the Royal College of Physicians (RCP) in 2005 and 2006 in a series of ‘road shows’. The findings from these events were reported on in 2008 in a joint publication: “Understanding Doctors: Harnessing professionalism”.  To raise the issue of professionalism among the doctors of the future, we are working in partnership with the RCP, General Medical Council, the NHS Institute, the Centre for Excellence in Developing Professionalism, University of Liverpool, and the Student BMJ.
