The capital retains this function because of its concentration of undergraduate and postgraduate teaching hospitals.  However, the shift of population away from the city's centre over the past century has created problems of access which remain pertinent today.  The paper makes a detailed examination of changes in the pattern of specialist services since the London Health Planning Consortium recommended rationalisation and dispersion from the city centre in the early 1980s.  In practice, it concludes that a further expansion and concentration of most specialist services has taken place in inner London between 1980 and 1990.  However, the paper argues that the changes brought about by the NHS and Community Care Act 1990 may result in changes to traditional care patterns.  It was prepared to inform the work of the King's Fund Commission on the Future of Acute Services in London. A specially designed simulation exercise examined the dilemmas facing purchasers in the new NHS. Organised over 24 hours, the simulation centred on three case studies: options for tackling coronary heart disease; the treatment of stroke in elderly patients; the balance of spending between different services and care groups. The aim of the simulation was to explore at an early stage in the development of the purchasing role the challenges facing health authorities in setting priorities. It also raises the most potential difficulties in implementation.  The study falls into two parts.  The first is a description of the preparatory period from January 1990, soon after practices were asked if they were interested in joining the scheme, to April 1992 when the practices took on their budgets. The aims of the survey were: to determine current interests and work patterns at the King's Fund in relation to Europe; to assess the need for more European material in the Centre Library. The results of the survey show that a high level of interest was shown across the Fund in health care in all regions of Europe. Particular interest was shown in the European Community.  A limited amount of work was being conducted, primarily by Faculty and other research staff based at the Palace Court site. The work concentrated on aspects of health care in Central and Eastern Europe although some work was being done in other regions.  The type of literature required by staff to fulfil their work and interest in European health care issues was diverse, with all types of European documentation of interest.  Contacts of people and organisations were the most requested items.  Respondents generally preferred English language material, although some interest was shown in foreign languages.  The quality of the information provided was felt to be more important than its
