Aimed at encouraging the social inclusion of a highly vulnerable and marginalised group, Working Together in London will establish assertive outreach teams to work with severely mentally ill people. It will also seek to integrate the work of the teams with a range of local services - covering health care, social care, housing, training, employment and benefits. This research review was commissioned to help develop the thinking behind the initiative. The picture it paints of a lack of integration between the urban regeneration agenda and local mental health services is striking. It also points to the considerable potential to bring the two issues together and tackle the social exclusion of severely mentally ill people in our inner Its central theme is that the widely differing perspectives of those who plan, fund, deliver, receive, record and administer care may each be valid in particular contexts, and a holistic approach to quality must incorporate these multiple perspectives. This paper recognises the plurality of perspectives that are already beginning to exist. It begins by describing the historical and political context for the current drive for quality in UK primary care. It then illustrates the notion that the core features of general practice remain difficult to define and measure and argues that the reflective practitioner should be conscious of the tendency of quality assessment to measure the easily measurable and exclude the essence of good general practice. A number of individual perspectives on quality are then explored. The different potential approaches for using quality initiatives as levers for change in general practice are then discussed, with the authors describing their own work to develop a methodology for defining, measuring and improving quality at a local level and arguing that extensive dialogue with key stakeholders is necessary when standards are being set. This document suggests a five part framework comprising the following areas: practice development, service development, professional development, organisational development and strategic development. It discusses various definitions of primary care and of quality in primary care; some of the key features of London's population particularly relevant to primary care; characteristics of primary care in London; service development in primary care; the professional development of those working in primary care; organisational development in primary care; and strategic development in primary care. It draws conclusions and makes recommendations for early tasks for PCGs. Although PCGs are now at the centre of primary care development , this agenda operates at different levels - practice level, PCG level, supra-PCG level, and pan-London level. The findings are based on an analysis of focus group discussions held in three different parts of the country involving service users, carers and front-line staff with experiences of health, housing and social care services. The findings were that people with long term illness or disability wanted to be able to lead an 'ordinary life'. They were concerned about obtaining appropriate services when needed and having ways of coordinating services to ensure ease of access and continuity of care. Information was seen as the key to the provision of good services and the way people with disabilities are treated can make the difference between feeling respected or rejected as an equal citizen. People talked about how they used their own money to meet costs associated with disability, how they were affected by current expenditure constraints and how they felt about the way public money was being distributed. There was a cynical attitude towards charters. The report concludes that action is needed to improve service performance and a new charter will make a valuable contribution. Ten essential tasks have been identified which when used together as components of an implementation programme can lead to success. Effective local arrangements for clinical governance will require organisations to put in place systems to assure the delivery of high quality health care.
